5.__Forms; rules.__The state registrar shall develop by rule the forms as required by this section and may adopt other rules for the administration of this section.__Rules adopted pursuant to this subsection are routine technical rules as defined in Title 5, chapter 375, subchapter 2-A.

Sec. 6. Effective date. This Act takes effect January 1, 2007.

SUMMARY

This bill establishes a process by which an adult adopted person may obtain a copy of that person's original, unaltered birth certificate. This bill also allows a birth parent to include with the child's original birth certificate a form that indicates whether the parent wishes to be contacted by the child and a medical history form.
