§14553.__Registration requirements

1.__Forms; information required.__The commissioner shall adopt forms for contractor registration that require, at a minimum, the following information:

A.__The contractor's name, address, telephone number and any other communication access numbers;

B.__A statement detailing the contractor's experience in the trade;

C.__Proof that the contractor has workers' compensation insurance coverage; and

D.__Proof that the contractor has liability insurance coverage.

2.__Fees.__Each contractor registering under this section shall pay an annual registration or renewal fee established by the department by rule pursuant to Title 5, chapter 375, subchapter II-A at a level sufficient to cover the costs of administering this chapter.__Rules adopted pursuant to this subsection are routine technical rules.

§14554.__Information provided to applicants

When a contractor requests a registration form, the department shall provide that contractor with an information packet that sets out all requirements under state law with which the contractor must comply, including licensing requirements, tax liabilities, insurance coverage, labor laws, consumer and contract requirements and professional codes of ethics or conduct.

§14555.__Renewals; updates

A contractor shall renew the registration annually, at which time the department shall provide that contractor with applicable updates or amendments to the information provided under section 14554.

§14556.__Coordination with licensing boards

The department shall coordinate registration under this chapter with any other licensing or certification requirements applicable to a contractor specific to that contractor's trade or
